Continuum, bringing the highest standards of care from the hospital to the community

Summary


Up to 40% of patients hospitalized for major cardiovascular events (myocardial infarction, heart failure, heart surgery) are diabetic. During this episode of care, new goals for cardiovascular treatment and prevention are generally set for the patient in accordance with Canadian recommendations in cardiology and diabetes. Unfortunately, these objectives are rarely achieved due to the lack of close support, the patient’s encouragement to take care of himself or even just the right information. The current challenge in a context of limited human resources is to be able to offer this supervision to an ever increasing number of patients.
